Note that ALL PRINCIPAL INVESTIGATORS MUST BE FULL TIME, PERMANENT SEU FACULTY OR STAFF.

Animal Use Protocol: use this form to submit new protocols for review by IACUC and to renew protocols previously approved by IACUC. Investigators must copy their Dean and Chair when submitted to IACUC.  Animal_Use_Protocol_SEU

Field Observational Studies Protocol: use this form to submit new field observational protocols (no handling of wildlife) for review by IACUC and to renew field observational protocols previously approved by IACUC. Field_Observational_Protocol_SEU

Annual report: investigators are required to submit a report for each active protocol annually.  Protocol-Annual-Report-SEU

Protocol  Modification Form: this form must be submitted and approved by IACUC in advance of any changes to the approved protocol.  Changes that need to be approved include a change in number of animals used, in how they are collected or housed, or changes to experimental procedures.  Changes in personnel working on the project should be documented in Protocol Personnel Modification form below.  Protocol Modification Form
